- All service must be prepaid for one year.
- Shipments are not made to accounts with negative cash balances.
- Cancellations or changes to the account require 45 days written notice and no refunds will be given.
- OSL dosimeters remain the property Radiation Safety, llc and must be returned. Unreturned, lost or damaged badges are subject to a $50 charge 60-days after the wear period ends.
- Fee’s and expense incurred for collections (including attorney fee’s) will be the sole responsibility of the purchaser of the service from Radiation Safety, llc.
- Legal proceedings will be in the state of Florida.
- Late fee’s will incur a 1.5% fee charged per month on the outstanding balance in accordance with the USA Patriot Act of 2001.
- You authorize Radiation Safety, llc to to use the card on file for automatic billing of yearly subscriptions, new users and any penalties on the account including fee’s for lost, damaged or late OSL radiation detection badges.
